Derek and Amelia work together on conjoined twins and Derek has a tough time giving Amelia the respect she has earned as a surgeon.  To be fair, McDreamy did see her through her rebellious teen years and a couple of drug addictions...

However, Derek is not only concerned with Amelia during the surgery.  He demands that if anyone cannot handle bieng involved in the surgery, they should step down from the project.  He essentially calls out the younger residents who have yet to prove themselves as outstanding doctors...

On the opposite side of the spectrum, Cristina is invited to go to Switzerland to speak about her work at a medical conference.  Meredith pushes her to go because she thinks it will be good for Cristina to have a change of scenery.  Unfortunately for Cristina, the change of scenery brings out a blast from the past: Doctor Preston Burke, a.k.a. the man who left her at the alter and broke her heart.

Back in Seattle, Miranda's patient that she secretly injected with a de-activated HIV strain, Brandon, starts to recover.  The parents and doctors are amazed that he's recovering so well, but Miranda secret behind the miracle.  Stephanie notices the change in the wind and puts two and two together.  When she confronts Miranda, Miranda smugly says she has no idea what Stephanie is talking about.

While Stephanie is on her medical game, Jo is falling flat without Alex by her side in the hospital.  After Alex leaves the hospital for a private practice, he comes home every day ranting and raving about how great his new job is.  Jo constantly drops the ball at work and starts to question her abilities.  The real question she has to ask herself is whether or not she will be able to hold her own in the hospital without Alex's constant assistance...

Another couple facing hard times is April and Jackson.  With their first sonogram going awkward and unsettling when the technician asks if they plan on keeping the baby.  April is faced with more uncertainty when she hears how excited Arizona and Callie are about their new baby plans.  Jackson eventually tells April that he may not share her religion, but there's no way he'll be sitting at home when his family is at church every Sunday.  This puts April at ease for now.

Uneasy in Switzerland with Preston, Cristina learns that the hospital she has been speaking at is Preston's brain child.  Independently funded, the hospital specializes in cardiology.  Equipped with 50 3D printers and holographic surgical tools, the hospital is Cristina's dream come true.  He gives her a tour that enchants Cristina and she confesses her latest goal she has in mind to him: printing a working, human heart.

After his enchantment, Preston invites Cristina to dinner.  Being uncomfortable and seeing the ghost of her past trying his hardest to woo her to work at his hospital, Preston confesses he has a wife and a child.  He tells her that his wife was a surgeon but gave up her career to raise their child.  Sounds like a broken record of expectations for Cristina...

Cristina tells Preston she could never work for him, especially with their history.  It's obvious the two of them still have a connection even though they have both moved on.  He tells her that their love was an all consuming love and it nearly destroyed them both.  After his confessions, he reveals to her his true intentions: He wants to give her his hospital.

When Cristina returns to Seattle, Meredith knows that Cristina will be leaving her.
